The Position Health platform enables providers to prevent readmissions and reduce total cost of care for their patients.

We use geolocation on patient smartphones to detect when a patient enters an emergency room anywhere in the U.S. The platform then generates an alert to the patient's previous provider and enables a call to the ER treating the patient.

This phone call has been proven to reduce costs, improve continuity of care, and prevent readmission.
